What is in the NAD+ & B12 pen?
This is the Synedica NAD+ & B12 pen, supplied sealed and unopened exactly as it leaves the manufacturer. What is NAD+?
What is NAD+? Nicotinamide adenine dinucleotide is a coenzyme present in every living cell. It exists in two forms, oxidised (NAD+) and reduced (NADH), and cycles between them as it carries electrons in the reactions that turn nutrients into usable energy, which is why it sits at the centre of glycolysis, the citric acid cycle and mitochondrial respiration. Beyond its redox role, NAD+ is consumed as a substrate by three families of enzymes that have made it one of the most studied molecules in ageing biology: the sirtuins (SIRT1 to SIRT7), which regulate gene expression and mitochondrial function; the PARPs, which repair DNA; and CD38, an NAD+-consuming enzyme whose activity rises with age.
The finding that cellular and tissue NAD+ levels fall substantially with age, reported in rodents and in human tissue samples, has driven a large research programme into restoring it: through precursors such as nicotinamide riboside (NR) and nicotinamide mononucleotide (NMN), through direct NAD+ administration in animal models, and through inhibition of CD38. That literature is what a nad+ research reagent is for. It is often listed by vendors as an NAD+ peptide; strictly it is a dinucleotide coenzyme rather than a peptide, but the term has stuck. It is not a description of what this pen does in a person; NAD+ has no marketing authorisation as a medicine in the UK for any indication.
What is vitamin B12, and why is it paired with NAD+?
Vitamin B12 (cobalamin) is a cobalt-containing vitamin and the cofactor for two enzymes: methionine synthase, which regenerates methionine from homocysteine and feeds the methylation cycle, and methylmalonyl-CoA mutase in mitochondrial fatty-acid and amino-acid metabolism. Its research interest alongside NAD+ comes from the overlap between one-carbon metabolism and NAD+ metabolism: the salvage pathway that recycles nicotinamide back to NAD+ consumes methyl groups, and both systems converge on mitochondrial function. Manufacturers pair the two for that reason; whether the pairing has any additive effect is not established in any controlled study, and the combination is supplied here for research into both compounds, not on the basis of any claim about the pair.

The two formats on the market are a lyophilised NAD+ vial, usually 500 mg or 1,000 mg of powder to be reconstituted, and a pre-filled pen like this one. The vial gives a laboratory control over concentration and diluent; the pen gives a fixed, manufacturer-prepared solution in a sealed cartridge with dial-metered delivery, and removes reconstitution as a variable. NAD+ vs NMN is a common comparison. NMN (nicotinamide mononucleotide) and NR (nicotinamide riboside) are precursors: the cell converts them into NAD+ through the salvage pathway, and they are sold as oral supplements. NAD+ itself is the finished coenzyme. In cell-culture and animal work the distinction matters because the precursors depend on transporter and enzyme availability in the tissue studied, whereas NAD+ supplied directly bypasses those steps; in oral use the intact NAD+ molecule is largely broken down in the gut, which is why NAD+ products for research are supplied as solutions rather than tablets. None of this is guidance for choosing anything for use in a person.
